Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting Community Health Nurses to join the 0-19 Public Health Nursing Team, supporting the Healthy Child Programme and early interventions around parenting and behavior. You will work with Children and Families to deliver core contacts and high impact areas.

As part of the team, your role includes assessing needs, promoting wellbeing, and liaising with Health Visitors and Nursery Nurses.

Responsabilidades

• Meet the needs of patients and families to improve health outcomes.
• Promote health and wellbeing with care, compassion, competence, and communication.
• Implement and evaluate packages of care within the Healthy Child Programme.
• Work in partnership with other agencies and families to innovate with children and families.
