Job summary

NHS Lothian is seeking Health Care Support Workers for the East Lothian Hospital to Home Team. You will join a new dedicated group to support people to stay longer at home, in partnership with local health and social care services.

Responsibilities include assisting with activities of daily living, medication support, meal preparation, and palliative care, in a community setting, with shifts across early mornings and evenings. A full driving licence is essential for this role.

Qualifications

• Full driving licence required.
• Experience in caring for elderly in community setting preferred.
• Ability to work flexibly including early mornings and evenings.
• Ability to assist with activities of daily living and be safe with medication.

Responsibilities

• Assess, plan, provide and monitor nursing care in a community setting.
• Support with activities of daily living, medication, meal prep and end of life care.
• Collaborate with East Lothian Council, NHS PCCC and voluntary services to support patients at home.
• Maintain patient safety and dignity during all interactions.
