Job summary

Buckinghamshire Healthcare NHS Trust is seeking enthusiastic Healthcare Assistants to join our Community Nursing Service. The role supports care delivery across Buckinghamshire, providing direct patient care in the home and assisting with long-term condition reviews, phlebotomy, and continence care under supervision of Community Nurses.

The post offers ongoing training within a 7-day service. You will work with a supportive team to help people remain independent at home, aim to avoid unnecessary

Qualifications

Responsibilities

  • Work alone using initiative within the guidance of the team.
  • Prepare for and provide care at a patient’s home per the care plan.
  • Take part in experiential training schemes in the Adult Community Healthcare Team.
  • Undertake training to acquire skills for development in the post.
  • Deliver care with respect for privacy, dignity, preferences and choices.
  • Obtain and understand consent for interventions.
  • Develop relationships with patients and carers to support goals.
  • Communicate patients' health status to the Case Manager.
  • Being a car driver and owner is essential.

Job description

Do you want to make a real difference to patients in their own homes while developing your healthcare skills? We are looking for enthusiastic Healthcare Assistants to join our Community Nursing Service in a new role that supports care delivery across Buckinghamshire. Working as part of a supportive team, you will provide direct patient care and assist with long-term condition reviews, phlebotomy, continence care and other delegated clinical tasks. You'll work closely with Community Nurses and other healthcare professionals to help people remain independent at home, avoid unnecessary hospital admissions and achieve the best possible outcomes. This is an excellent opportunity for Healthcare Assistants who are keen to develop their clinical skills and be part of an innovative approach to community healthcare. We offer ongoing training and support within a 7-day service.

Key responsibilities
  • To work alone using initiative and a wide range of health and social care skills, within the guidance set and agreed by the professional members of the team. The term 'professional' member of the team refers to those clinicians, social workers etc within the team who make the initial assessment and are responsible for the patient's case management.
  • To prepare for and provide care and interventions enabling patients in their own home or community based facility. You will follow the care plan written by the patient's case manager.
  • To take part in the Adult Community Healthcare Team's experiential training scheme
  • To undertake training to acquire the skills appropriate for development in the post
  • To deliver care in a way that understands the differences in values and beliefs held by the patient and supports their right to privacy, dignity, preferences and choices.
  • To obtain and understand the importance of consent for the patient in all interventions.
  • To develop relationships with patients and carers who value them as individuals,communicating effectively to promote the patients goals.
  • To use initiative and be responsible for clear communication of patients health statusto the Case Manager.
