This is an exciting opportunity for an enthusiastic, inspiring, and reliable Community Health Care Assistant, who is willing to support and deliver a range of high-quality nursing interventions in the out-of-hours period.

Part-time position - 15 hours per week.

CWP has adopted the Living Wage. Where the standard Agenda for Change hourly rate is below the Living Wage, pay will be supplemented to this level.

Main duties of the job

Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person-centered approach. This involves providing quality care to patients in their own homes or in long-term care settings, aiming to avoid unnecessary hospital admissions.

Since the role involves visiting patients in their environments, access to transport is essential.

Qualifications
  • Related qualification or willingness to complete an apprenticeship relevant to the Healthcare Assistant role.
  • Completion of or working towards an apprenticeship and qualification at level 3 or above in a relevant subject.
  • Ability to demonstrate work procedures, practices, knowledge, and skills.
  • Completion of or commitment to complete the Care Certificate (CWP in-house 2-day training within 3 months).
Knowledge and Expertise
  • Understanding of confidentiality, empathy, and sensitivity.
  • Basic wound management.
  • Basic risk assessments and record keeping.
Experience
  • Providing quality care while adhering to Trust policies, working towards the Care Certificate.
  • Experience working collaboratively with colleagues, teams, and service users.
  • Supporting others in care settings, whether professionally or through lived experience.
  • Basic clinical skills such as venepuncture, skin care, wound care, and clinical observations.
  • Experience in community care, including personal care provision.
