Job description

We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant, who is keen to deliver a range of quality care services to patients visited by the Crisis & Reablement Team based at Civic Way, Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.


An enthusiasm to work as part of a team which delivers high quality services is a must for this role and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be flexible, reliable and approachable.


The hours are 27 per week, consisting of 3 x 9 hour shifts, worked on a rota basis between 22.00 – 07.00 including Bank holidays and weekends.

As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves lone working providing overnight care in patients' homes. This post requires travel across the Western Cheshire footprint; access to transport for work purposes is essential.


CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
