Job summary

Betsi Cadwaladr University Health Board (BCUHB) is seeking a Band 3 Health Care Assistant to join the Community Resource Team in Wrexham. This 36-hour-per-week post involves flexible shifts from 8am to 10pm, weekend work and bank holidays, with travel across the county to support patients at home.

You will assist with daily living activities, promote independence, and work with a multidisciplinary team to deliver individual care plans.

Qualifications

• Experience in health or social care with direct patient care.
• Level 3 health care qualification or equivalent with demonstrated competency.
• Knowledge of safeguarding adults/children and GDPR.
• Enhanced DBS clearance including adults and children’s barred list checks.

Responsibilities

• Assist patients with daily living activities to promote independence.
• Support patients at home with personal care such as washing, dressing and meals.
• Work with multidisciplinary team and external agencies to meet needs.
• Travel across a large geographical area to meet service requirements.
