We are seeking to appoint an enthusiastic, motivated, and reliable Community Health Care Support Worker to deliver quality care services to patients visited by the Community Health Support Team based at Chowley Oak Tattenhall. The team serves patients across Western Cheshire within the Integrated Community Care Teams.
The ideal candidate should have experience working in patients' homes and be capable of tasks such as simple dressings, catheter care, personal care, and recording blood sugars. The role involves some lone working, providing care to those in their final days or weeks of life, supporting patients to prevent hospital admissions, and facilitating hospital discharges.
This position offers a pay rate supported by the Real Living Wage (RLW) as set by the Living Wage Federation, with additional pay supplements if the national A4C hourly rate falls below this.
Travel across the Western Cheshire footprint is essential, and access to personal transport for work purposes is required.
