Job description

This is an exciting opportunity for an enthusiastic, inspiring and reliable Community Health Care Assistant, who is willing to support and deliver a range of high quality nursing and phlebotomy services to patients in their own homes who are registered with GP Practices within Chester South.

A commitment to support our team in delivering high quality services is essential for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, trustworthy and approachable.

Previous experience in this role would be an advantage. Travel is an integral part of this post.

Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital.

As the role involves visiting patients in their own environment, access to transport for work purposes is essential.

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.

We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton and Liverpool. We also provide specialist services for the North West as a whole.
