Overview

Mid Cheshire Hospitals NHS Foundation Trust is looking for an experienced Band 3 HCA to join a busy community team. The post is based in Sandbach, working within SMASH care community. The closing date is 29 September 2025.

The role involves caring for patients who are approaching the end of life. The post holder will undertake clinical tasks as delegated by the Community Nurse team and within their sphere of competency. You will work without close supervision, using your own initiative while following the patient’s care plan. The successful candidate should be passionate about making a difference to people’s lives, helping them to live as well as possible and to die with dignity. You will need good time management, organisational skills, and flexibility to work in other areas if required to meet service needs. Excellent communication skills and emotional strength are essential, with the ability to build empathetic relationships while providing palliative care. Ongoing training and support will be provided.

Main duties of the job

Your role will include delivering a range of nursing interventions to patients in their own homes. This includes wound care, phlebotomy, medication administration and risk assessments.

  1. Function as an effective member of the multi-disciplinary team
  2. Maintain clear, concise and legible documentation adhering to prescribed standards and protocols
  3. Use IT systems for relevant tasks (e.g., accessing patient records & results, data collections and inputting data)
  4. Obtain consent in accordance with Trust Consent Policy
  5. Report accidents/incidents on the clinical incident reporting system for patients or self while on duty
  6. Maintain a patient-orientated approach towards care delivery, promoting a rehabilitative and self-care approach
  7. Undertake phlebotomy duties as delegated and overseen by the registered nurse, working within Trust policies and procedures
  8. Undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency
  9. Assist the Community Nursing Team in management of patients with Long Term Conditions and End of Life care
  10. Recognise the need for referral and liaise with other health care and multi-agency workers as necessary
  11. Participate in induction and orientation programmes for all grades of staff requiring community experience as arranged by the Team Leader
  12. Maintain up-to-date knowledge of blood tests performed and prepare appropriate sample tubes; ensure correct labelling at the time of sampling
  13. Maintain phlebotomy stocks, keeping them clean and in good order
  14. Follow Trust policy for needle-stick incidents and ensure accurate reporting on the Ulysses system; dispose of syringes, needles, etc., in accordance with health and safety procedures
