Community GP, Team Up Derbyshire (2 sessions - Fridays)

Do you want analternative to the current model of General Practice, where you can spend moretime with the most complex patients?

South Hardwick PCNare seeking to recruit a Community GP to support our multi-disciplinary team providinghealthcare to our housebound patients and patients in care or residentialfacilities across the PCN footprint.

This is an ideal role for a portfolio GP with negotiable sessions available.

This is an excitingopportunity to be at the forefront of the delivery of our Ageing Well modelof care - Team Up Derbyshire. The role transcendsthe traditional health and social care organisational boundaries. It willinvolve true MDT working, building relationships with GP practices, Community Core and Rapid Response Nursing and Therapy services, Falls services and Adult Social Care, to help clinically deliver theservice on a day-to-day basis.

This is an evolving service,so you will have the opportunity to contribute to the development of Team Up andthe Home Visiting Service if you wish to.

The aim for the service isto ultimately provide a holistic approach to acute on-the-day/rapid responseservices, enhanced health in care homes and enhanced proactive care for olderpeople with frailty and patients with multi-faceted health problems.

Main duties of the job

Key Duties Tasksand Responsibilities

Provide day-to-day clinicalleadership for the multi-disciplinary team including debriefing after visits ifneeded

Maintain a visible clinical profile, having both advisory and clinical input into the medical management of patients presenting to the service

Home visiting for clinicallycomplex cases e.g. end of life care, diagnostic uncertainty, complexprescribing decisions

Provide senior clinical triage forthe Acute Home Visiting service

Maximise best care in the patients' own homes to reduce the need for hospital or care home admissions

Build relationships acrossthe extended team including, but not limited to social care, ambulance, socialprescribers, community, and mental health clinicians.

Lead/Engage in service development as the team expands

Lead/Engage in MDT meetings

Provide holisticcomprehensive assessment and enhanced advance planning

Maximise best care in thepatients own homes to reduce the need for hospital or care home admissions.

Provide end of life care inline with the patients wishes

Maximise opportunities for teachingand mentorship
