West London NHS Trust is seeking a Support and Recovery Worker to deliver care to a caseload in the Community, focusing on Early Intervention in Psychosis. You will work within a multi-disciplinary team and provide support to service users and carers in the community.

Formación

• Experience in a community-based setting.
• Ability to work within an Early Intervention framework.
• Collaborative approach within a multi-disciplinary team.

Responsabilidades

• 1. To work within the multi-disciplinary team structure of the Adult Early Intervention in Psychosis Team (EIS).
• 2. To support and work alongside the Care Co-ordinators and to assess, plan, implement and evaluate individual programmes of care.
• 3. To provide Support, give Time to an allocated group of service users, promoting Recovery and maintaining them in the community environment.
• 4. To make a valuable contribution to multi-disciplinary team assessments and the formulation and implementation of MDT care plans.
• 5. To maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
