Descrizione del lavoro

An exciting opportunity has arisen for suitably experienced candidates who are wanting to work in our newly developed Ealing Intensive Outreach and Rehabilitation team with two arms one being Rehabilitation & the other Assertive Outreach. Ealing is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users with complex psychosis who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support., The post holder will be working in a Specialist Outreach Psychosis Service within the Assertive Outreach Pathway, taking responsibility to assessing, planning and coordinating the care plan and treatment of a defined caseload of service users with complex needs related to psychotic illnesses who are difficult to engage.

The Specialist Outreach Psychosis Service is a multidisciplinary, community-based service responsible for providing on-going holistic assessments, treatment and support to clients with severe and enduring mental illness living in the community. The service will enable patients with severe mental health problems to be supported and cared for in the community in the least restrictive setting through an assertive, personalised and integrated approach to care and treatment.

The service will provide an intensive community-based mental health service delivered in service users' home environments, in the community and in supported living accommodation or other locations agreed with the service user utilising "A whole system approach" to recovery from mental ill health. The approach aims to maximise an individual's quality of life and address social inclusion by supporting engagement, encouraging their skills, promoting independence and autonomy to give them hope for the future which leads to successful community living through appropriate support.

Successful candidates will show knowledge and experience of engaging people in the community who are experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.

Experience of working with complex mental health needs and clients that present as high risk is essential as well as strong skills in engaging people, being flexible in your approach, a sound knowledge of community resources, care planning and crisis management. Experience of community based and outreach working is also essential
