What you will do:

  1. Conduct thorough assessments to develop effective diet plans for community patients, with a focus on long-term conditions such as type 2 diabetes and obesity.
  2. Provide expert nutritional advice and tailor educational programs to meet the diverse needs of patients.
  3. Collaborate with multidisciplinary teams to ensure cohesive and comprehensive care for patients within the community.
