Job summary

HCRG Care Group seeks a Community Dietitian to deliver high-quality, patient-centred nutrition care across Wiltshire and Swindon in community settings. You’ll assess, treat and manage a varied caseload with evidence-based plans, supported by a collaborative team and ongoing CPD.

Qualifications

• Degree in Nutrition and Dietetics (or equivalent for HCPC).
• Evidence of ongoing Continuing Professional Development (CPD).
• Experience in NHS inpatient/outpatient dietetic services.
• Experience using MUST or similar nutrition screening tool.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Manage a diverse clinical caseload, delivering high-quality dietetic assessment, diagnosis, treatment and review.
• Develop and implement evidence-based nutritional care plans.
• Communicate complex information to patients, carers and professionals using behaviour change techniques.
• Provide personalised dietary advice tailored to individual needs and goals.
• Collaborate with GPs, community teams, care homes and other healthcare professionals to deliver integrated care.
• Assess need for oral nutritional supplements and advise on nutrition-related prescribing.
• Support nutrition and hydration initiatives across community and inpatient settings, including menu reviews.
• Design and deliver training on diabetes, nutrition risk screening, pulmonary rehabilitation, and healthy eating.
• Maintain accurate electronic records and contribute to audits and quality improvement.

Join our established Community Dietetic Team and play a vital role in delivering high-quality, patient-centred care across Wiltshire and Swindon. Every day, you'll help individuals achieve better health outcomes, maintain independence, and improve their quality of life through expert nutritional support in a variety of community settings.

As a Community Dietitian, you will provide high-quality, evidence-based dietetic assessment, treatment and management to patients across a range of community environments. Working as part of a supportive and experienced team, you will deliver personalised nutritional care to individuals with diverse clinical needs while contributing to innovative and responsive community services.

This is an exciting opportunity to develop your clinical skills across a varied caseload while making a meaningful impact on patients' lives outside of traditional hospital settings.

Contract: 37.5 hours per week (Full-Time)

What You'll Be Doing

• Managing a diverse clinical caseload, delivering high-quality dietetic assessment, diagnosis, treatment, and review for patients across community inpatient and outpatient settings.
• Developing and implementing evidence-based nutritional care plans, using clinical judgement to interpret biochemistry, anthropometric measurements, dietary intake, and medical information.
• Communicating complex nutritional information effectively to patients, carers, and healthcare professionals, using behaviour change, motivational interviewing, and counselling techniques to support positive outcomes.
• Providing personalised and practical dietary advice, ensuring interventions are tailored to individual needs, circumstances, and treatment goals.
• Working collaboratively with GPs, community teams, care homes, support workers, and other healthcare professionals to deliver integrated, patient-centred care.
• Assessing the need for oral nutritional supplements and provide specialist advice on nutrition-related prescribing and food-drug interactions.
• Supporting nutrition and hydration initiatives across community and inpatient settings, including menu review, nutritional standards, and partnership working with catering teams.
• Designing and delivering training and education programmes for healthcare professionals, care home staff, and patient groups on topics such as diabetes, nutrition risk screening, pulmonary rehabilitation, and healthy eating.
• Maintaining accurate and contemporaneous electronic patient records using SystmOne, contributing to audit, service evaluation, clinical governance, and quality improvement activities.
• Contributing to a culture of continuous improvement by participating in professional development, appraisal, risk management, service development projects, and the efficient use of organisational resources.

What We’re Looking For

• Degree in Nutrition and Dietetics (or equivalent qualification acceptable for HCPC registration).
• Evidence of ongoing Continuing Professional Development (CPD).
• Experience working within inpatient and outpatient dietetic services in an NHS setting.
• Experience using MUST or a similar nutrition screening tool.
• Understanding of the responsibilities and requirements of a Community Dietitian role.
• Experience working effectively within a multidisciplinary team.
• Excellent verbal and written communication skills.
• Strong presentation and education skills.
• Effective problem-solving, organisation, and time management skills.
• Ability to work independently using initiative, as well as collaboratively as part of a team.
• Enthusiastic, positive, and committed to continuous learning and development.
• Good IT skills and confidence using digital systems.
• Self-motivated, adaptable, and able to manage changing priorities.
• Reliable, organised, and attentive to detail.
• Full UK driving licence and access to a vehicle for work-related travel.
• Experience providing clinical supervision or supporting the development of others.
• Membership of the British Dietetic Association (BDA).
• Training or experience in behaviour change techniques and motivational interviewing.
• Previous experience working within a community dietetics service.
