A community health provider in Barnsley is seeking a Band 5 Community Dietitian to join its busy Nutrition & Dietetics Service. This role offers a varied caseload across various settings, including care homes and clinics. We are looking for anenthusiastic Band 5 Dietitian to join our busy Community Nutrition &Dietetics Service in Barnsley, for 1 year from September 2025 (fixed termcontract).
The post is varied andinteresting, covering a range of community dietetics including communityclinics, care homes and people who are housebound. The caseload includesnutrition support & general dietetics, with the opportunity to gain someexperience with diabetes & enteral feeding (with support and supervision)

You will need excellent teamworking, communication, organisation and time management skills, as well as beable to demonstrate a commitment to CPD.
This is acommunity-based role; applicants must have a driving license and access to avehicle for work purposes. Acontribution to the training of student dietitians is also expected.

As a Band 5 Community Dietitian you will:
Manage a caseload requiring dietetic intervention in the communitythis may include a range of settings such as care homes, clinics and patientsown homes.
Be able to provide dietetic assessment and intervention for a range ofconditions including nutrition support and general dietetics.
Work as a part of our team of dietitians and dietetic assistants.
Have a patient centred approach in line with our Trust values.
Work flexibly to meet the needs of the service.
Possess excellent communication/interpersonal skills to negotiate andbuild rapport with patients, carers and families.
Work with key members of multidisciplinary team (MDT) to provideindividualised advice, support & care plans to patients.
Participate in education & staff development, clinicalsupervision and the delivery of dietetic student training.

Clinical
1.Tobe professionally and legally responsible for all aspects of own work includingthe management of patients in your care
2.Toundertake clinical assessment and treatment of own caseload of patientsincluding calculation of individual diets and enteral feeding regimens whereappropriate
3.Monitorand review individual treatment plans as required according to patientsclinical condition to provide the most effective and efficient outcomes forpatients.
4.Organise,produce, deliver and evaluate training on nutrition and dietetics relatedissues other health service staff and patients and carers
5.Tocommunicate effectively with patients carers, members of the multidisciplinaryteam and other agencies to achieve patient centred goals
6.Toproduce and update dietetic/nutritional teaching materials and therapeutic dietsheets.
Professional
1.To beresponsible for maintaining own professional development through a variety ofCPD activities and reflective practice
2.To assistsenior clinicians with the supervision, training and induction of studentdietitians, dietetic assistants and newly appointed staff when necessary
3.To contributeto audit and development and implementation of clinical standards/policies/outcomemeasures within the department.
4.Respondto nutritional queries from other health professionals and the general public.
5.Tokeep accurate and comprehensive records in accordance with trust and dieteticdepartment standards, including computer based data entry.
6.Toadhere to Trust and dietetic department standards/policies and procedures.Including Trust and area specific appraisal and clinical supervision
7.Toensure that equipment such as enteral feeding pumps is used safely by staff,patients and carers.
8.Workclosely with immediate dietetic colleagues and team leader to provide acomprehensive and cohesive dietetic service.
9.Developand maintain links with other Dietitians, through local, regional and nationalnetworks.
10. To work collaboratively with serviceusers, catering staff and facilities monitoring co-ordinators to ensure thatmenus meet the nutritional, cultural and ethical needs of service users.
11. The post holder holder is expected tocover the duties of colleagues in posts within the range of the post holdersknowledge and skills.
