A leading healthcare provider in Bury St Edmunds is seeking a dedicated Nurse to join the Community Diabetes Team. The role involves managing Nurse Led Clinics and supporting diabetes patients through effective communication and care. Candidates should have significant experience and qualifications in nursing and a background in diabetes care. The position offers a salary of £47,810 to £54,710 per annum.
Exciting opportunity to work within the WSH Community Diabetes Team supporting patients with diabetes out in GP Practices. Opportunity to interface with primary care colleagues and build working relationships for the benefit of our patients. Requires a level of self-direction and autonomy to manage a specific caseload. Scope for both professional and service development to enhance this integration.
Candidate will run Nurse Led Clinics out in GP Practices managing patients with diabetes. Follow up of these patients will be via email, telephone and teams contact in between Clinic reviews. Ability to work autonomously with an organised approach is essential along with effective communication skills both with patients and HCP's across primary care.

STAFF CAR PARKING
Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 18:30.
For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.
This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete on call duties within their role.
