Job summary

The Surrey Downs Health and Care Diabetes Team is recruiting a Band 3 Healthcare Assistant to join the community team across Surrey Downs. You will assist in delivering effective care, support patients with diabetes, provide education, and share expertise on diabetes technology.

The team provides induction and training, works 8.30am–16.30pm, Monday to Friday, and offers opportunities for development. The post requires an interest in community-based care and working across multiple PCNs.

Qualifications

• Interest in working in a community setting.
• Ability to provide diabetes education to patients.
• Support clinics with basic checks and monitoring.

Responsibilities

• Provide patient education on diabetes management.
• Support clinicians in clinics with blood pressure, weight and glucose checks.
• Assist with downloading glucometer data and promoting health checks.
• Liaise with patients to improve safety and engagement with care.
