Job description

Location: South East Essex (Castle Point & Rochford and Southend), SS0 7DB

Job Title: Band 6 Dementia Nurse Practitioner

We are seeking a dedicated Band 6 Dementia Nurse Practitioner to join the South East Essex Dementia & Older Adult's Community Mental Health Service. The successful candidate will work with the Dementia Intensive Support Team, supporting individuals with or suspected of having dementia, and assisting in needs assessment and care planning to prevent unnecessary hospital admissions.

Key responsibilities include:

  1. Supporting early discharge and facilitating hospital discharges in collaboration with hospital teams.
  2. Working with a multidisciplinary team, including the Single Point of Referral (SPOR), EPUT Community Services, and Southend University Hospital.
  3. Providing short-term, effective support interventions to reduce hospital admissions and re-admissions.
  4. Supporting individuals and their carers in their own homes, care homes, or alternative community settings during periods of acute illness or rehabilitation.
  5. Assessing care needs and developing, implementing, and evaluating care programs for patients with dementia or suspected dementia.
  6. Collaborating with hospital, community, primary health, social care, and third sector professionals.
  7. Maintaining care standards, participating in staff training and education, and delivering person-centered care.
  8. Ensuring the provision of an integrated service with good inter-disciplinary working.

The post holder will be a valued member of the Dementia Services team, which includes the Triage & Locality Community Dementia Nurse Specialists and Care Home Team. They will work closely with colleagues to support patients and their families, ensuring high standards of care and effective teamwork.
