Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking a clinician to provide short-term, goal-based interventions for people living with dementia. You will work within the Older Adult Community Mental Health Team and collaborate with Dementia Advisors and Primary Care to reduce distress and promote wellbeing.

The role includes clinical assessments, documentation, and engagement in service development; hours are Monday to Friday 09:00–17:00 with potential for part-time or flexible

Qualifications

• Relevant professional qualification in Health or Social Care (e.g., nursing, OT, social work) with current registration.
• Mental health experience and assessment skills.
• Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006.

Responsibilities

• Direct clinical assessment and interventions for people living with dementia; provide advice to Dementia Advisors and Primary Care.
• Offer short-term, goal-based interventions to reduce distress and maintain wellbeing.
• Contribute to service development through outcomes, metrics, and promotion events.
