Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking nurses or occupational therapists to join our Community Dementia Service within the Older Adult Community Mental Health Team. You will deliver brief interventions to people living with dementia and their carers, and collaborate with Dementia Advisors and Primary Care.

The role includes weekly dementia assessments and contributing to service development.

Qualifications

• Nurse or OT with dementia care experience.

Responsibilities

• Provide brief interventions to reduce distress, maintain wellbeing and avoid crisis.
• Collaborate with MDT and Dementia Advisors; liaise with Primary Care as needed.
• Undertake routine dementia assessments and other OACMHT activities.
