Job description

Community Connectors work as part of an integrated, blended mental health offer to support people with emotional, social, and practical needs. They help access local, non-medical, and non-clinical services to improve confidence, social inclusion, and independence. This role involves working within a multi-disciplinary team alongside statutory, third sector, and NHS colleagues to support clients experiencing moderate to severe mental health difficulties.

Key attributes for this role include experience working with mental health issues, in-depth knowledge of local services and support networks, adaptability, independence, and resilience in challenging environments.

What the Service Provides:
  • Work within a Primary Care Network as part of a blended mental health team, working individually with people to co-produce plans and set goals.
  • Take a holistic approach, considering wider determinants of health.
  • Offer in-depth knowledge of local VCSE and community resources.
  • Support individuals on a 1:1 basis over 12 weeks to overcome barriers to engagement and walk the journey together.
Service Delivery:
  • Assess suitability for community connection support and support individuals to access services using local voluntary and community networks.
  • Work with individuals to co-produce a plan addressing their mental health and wellbeing goals, facilitating access to appropriate resources.
  • Support people to access community resources, including accompanying them if necessary.
  • Provide guidance and help to build confidence, overcome fears, and address barriers to attendance.
  • Adopt a holistic approach based on individual priorities and wider determinants of health, such as debt, housing, unemployment, loneliness, and caring responsibilities.
  • Collaborate as part of an integrated team to ensure the best outcomes for each person.
  • Build relationships with community groups and relevant services for partnership work and signposting, keeping information up to date.
  • Work independently and flexibly within a blended team, supporting people with complex mental health challenges.
