Job description

Community CMSRT Rehabilitation Assistant - South Warwickshire

Responsibilities:

  1. Gain informed consent for treatment in accordance with Trust policy, often involving patients, carers, and relatives with diverse or complex conditions, requiring verbal and non-verbal communication tools.
  2. Develop and deliver therapy treatment programmes based on guidelines set by qualified therapists, including patient education, exercise programmes, or classes.
  3. Evaluate patient progress, reassess, and modify treatment programmes as necessary after discussions with qualified therapists.
  4. Ensure own competency before using medical devices (e.g., gym equipment, hoists, mobility aids) and when issuing equipment (e.g., appliances, splints, aids).
  5. Maintain accurate documentation both in writing and electronically, following professional, legal, and Trust guidelines.
  6. Manage own caseload within a defined service area, working independently without direct supervision.
  7. Maintain a clean, safe environment and equipment in Trust premises or clients' homes, respecting Health and Safety regulations.
  8. Apply clinical knowledge to assess and manage clinical risk within scope of practice.
  9. Follow infection control guidelines to prevent infection risks to staff, patients, and visitors.
  10. Support other therapy assistants as needed.
  11. Collect relevant clinical data for the service.

Communication:

  1. Communicate effectively with patients, carers, and relatives to maximize rehabilitation potential, using verbal and non-verbal methods, including explanation, instruction, persuasion, motivation, negotiation, and reassurance.
  2. Work collaboratively within a multidisciplinary team to ensure continuity and efficiency of service.
  3. Carry out therapy work within patients' home environments.

Other Duties:

  1. Order and maintain stocks of general store articles and equipment, liaising with managers for approval.
  2. Perform administrative tasks such as data input, typing, filing, and providing reception cover, in collaboration with clerical staff.
  3. Attend meetings as designated by senior therapists.
  4. Identify opportunities for service improvement to enhance patient care within available resources.
