Job description

The post holder will work as a specialised community dietitian to provide a dietetic service for outpatient clinics, covering a diverse caseload including nutritional support, frailty, weight reduction, diabetes, neurological conditions, gastrointestinal disorders, and respiratory conditions. The role involves collaborating with primary and secondary care teams to offer advice, support, and care plans tailored to patients within the specialist field. Additional duties include supporting the training of student dietitians and providing education and specialist advice to patients and health professionals. This position is ideal for a dietitian seeking to develop their clinical expertise in community dietetics within a supportive environment that fosters career growth.

Key responsibilities include:

  1. Delivering specialised dietetic services to patients using effective communication, counselling, and behaviour change techniques, addressing a wide range of medical conditions and nutritional diagnoses.
  2. Collaborating within a multidisciplinary team to plan and implement patient care, liaising with other disciplines and agencies as needed.
  3. Maintaining accurate, detailed records in line with local and professional standards, including automated systems, to enhance service quality and coordination.
  4. Supporting primary care requests and contributing to nutrition services that address health inequalities.
  5. Adhering to guidelines and policies to promote safe nutritional care, aligning with CQC and Trust standards.
  6. Participating in the development of quality standards and procedures, and conducting regular audits against departmental and Trust benchmarks.
  7. Providing educational sessions to staff (nursing, medical, AHP), catering teams, and patients.
