What is the job role?

An exciting opportunity has arisen for a Community Case Worker within one of our One Coventry Integrated Teams. These teams are built around localities to coordinate urgent responses tailored to community needs, including supporting hospital discharges and providing short-term reablement services.

Since no two cases are alike, you’ll enjoy a diverse caseload and opportunities to develop your skills. Our teams pride themselves on providing a supportive environment that fosters personal development and professional growth. This is a great time to join us as we expand within the Local Integrated Teams in partnership with health system providers.

This post is exempted under the Rehabilitation of Offenders Act 1974, and appointment will be conditional upon a satisfactory DBS check.

Who are we looking for?

We seek someone who is compassionate, resilient under pressure, self-motivated, and a team player. In this role, you will engage directly with community members, working closely with Social Workers, Nurses, Occupational Therapists, Physiotherapists, and other practitioners in a fast-paced, multidisciplinary environment.

With excellent relationship-building skills, you will collaborate with team members across various locations, including hospitals and care providers. Using a strengths-based approach, you will make confident decisions regarding residents’ rehabilitation, care planning, and independence promotion. Your proactive problem-solving skills will help achieve positive outcomes for individuals.

We offer a varied, fulfilling role where you can influence community services, work collaboratively with stakeholders, and leave a lasting impact in a dynamic multi-agency setting.
