Job Description:

Within the framework of Community Care legislation and professional guidance, undertake outcome-focused assessments and care management tasks for older people and their carers. This work will be done with access to regular supervision and advice. It does not involve decisions regarding legal powers leading to the loss of liberty of service users. Ensure that risks to service users and carers are identified and managed within policy guidelines.

Design tailored service packages to meet assessed needs, including aids to daily living, aligning with departmental policies and priorities. Agree on intervention objectives with individuals.

Negotiate effective ways to achieve these objectives and liaise with other professionals and agencies involved in care provision, incorporating them into individual care plans. Ensure copies of care plans are provided to service users and carers, and record actions appropriately.

Maintain close links and liaise with Department staff and other agencies (e.g., Housing, Health, voluntary, and independent sectors) to ensure an integrated service approach.

Oversee monitoring, reviewing, and reassessing care packages. Refer concerns about service provision (internal and external) to enable appropriate actions and ensure individual needs are met. Contribute to reviews, including multidisciplinary arrangements, and prepare necessary reports. Responsible for agreeing to variations in care packages within set parameters.

Be aware of the ethnic and cultural needs of the local population. Develop and deliver culturally sensitive care packages, seeking advice as needed.
