Job summary

Wiltshire Council is seeking a Social Care Practitioner to manage your own caseload and work with customers, carers and partner agencies to deliver person-centred care across the county.

You will collaborate with Social Workers and Occupational Therapists, promote independence and wellbeing, and use universal services and community resources. A sound understanding of the Care Act 2014 is required, along with NVQ Level 4 in Health and Social Care or progress towards it.

Qualifications

• NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification.
• Experience of working within a Health or Social Care setting.
• Sound understanding of the Care Act 2014.
