Job description

We are seeking a motivated and compassionate Band 6 Community Care Coordinator to join our Community Mental Health Team in Bradford.

This is an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centred mental health care. As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team. You will be responsible for assessment, planning, intervention, and evaluation of care, ensuring that service users receive safe, effective, and recovery-focused support.

Key Responsibilities:
  1. Coordinate and deliver community mental health care in line with Trust policies.
  2. Undertake comprehensive assessments and manage risk effectively.
  3. Work collaboratively with service users, carers, and partner agencies.
  4. Administer and monitor medication in accordance with NMC and Trust guidelines.
  5. Participate in case conferences and contribute to service development.
  6. Promote equality, diversity, and safeguarding in all aspects of care.
