Salary: £28,046 - £32,010
Hours: 36 per week, 52 weeks
We are looking for an enthusiastic individual to join our busy, supportive, and friendly locality team. You will work alongside Social Workers and Occupational Therapists, conducting assessments for equipment and minor adaptations, support assessments, and reviews within the community and home settings. Your duties will include assisting with minor property adaptations and equipment provision, supporting complex care planning, and aiding citizens with housing and social issues.
We offer hybrid working, which includes a mix of office-based, community, and home working.
Purpose of Job: To contribute to the provision and monitoring of services for individuals and their carers, adapting to changing circumstances within community and hospital teams.
Within defined boundaries, you will coordinate a range of social care services and equipment to meet assessed needs, helping individuals maintain independence, facilitate hospital discharges, and make choices about their quality of life.
This position involves regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The successful candidate will need to join or update the PVG Scheme and may require an Overseas Criminal Record Check if they have spent more than three months outside the UK in the last five years. Employment is contingent upon satisfactory completion of these checks.

Our salary range typically starts at the initial salary and increases annually until the top of the range is reached.
