Our primary goal is to provide the most appropriate support to individuals and families so people can remain in the comfort and familiar surroundings of their home environment, preserving their independence, dignity, and lifestyle choices after being in the hospital.

This is a new service to the area, aimed at alleviating pressure on the NHS by supporting those coming out of hospital and ensuring they receive the best support to reduce their care needs.

You will be the first responder, trained to a high level, to ensure you can deliver the best possible outcomes to our clients. You will deal with various clients, from those who just had surgery to those recovering from illness or injuries, helping them set and reach their recovery goals.

The role focuses on the client’s recovery and their goal to lead a happy, healthy, and independent life. Responsibilities include personal care (bathing, washing, toileting, medication) and motivating clients daily to meet their objectives and promote overall well-being. Support from Occupational Health and Physiotherapists will be available as needed.
