Job Description:

Position – Community Care Responder

Hours – Guaranteed 30 hours per week with shift patterns of 7:00am to 2:30pm and 3:30pm to 10:00pm, on a 2-week rolling rota with alternate weekends.

Our goal is to support individuals and families to remain in their homes, maintaining independence, dignity, and lifestyle choices after hospital stays.

This new service aims to ease NHS pressures by supporting hospital discharges and helping clients reduce their care needs.

You will be a trained first responder, handling clients recovering from surgery, injuries, or illness, assisting them in achieving their recovery goals.

The role focuses on client recovery and independence, involving personal care tasks (bathing, washing, toileting, medication) and motivating clients daily to meet their objectives, with support from Occupational Health and Physiotherapists as needed.

We have multiple full-time positions available, and you must be available to start immediately after recruitment checks.

Applicants must have access to their own vehicle or transportation and a valid driving license.
