The post offers the successful candidate a chance of caseload management in a community setting. Working one-to-one with patients referred into the service, the successful candidate will benefit from being part of a forward-thinking, dynamic team working towards the goals and outcome measures of the commissioning pack for Cardiac Rehabilitation.

Job responsibilities

The successful candidate will work the hours over 3 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is predominantly clinic-based but also includes home visits, telephone assessments, and in-reach to the acute service, working on the ward providing stage 0 of the commissioning pack pathway.

Person Specification

Experience

  • Two years post-registration experience, 1 of which must be in CHD care
  • Demonstrate leadership skills
  • Evidence of ongoing professional development
  • Experience of implementing research-based practice
  • Experience of working within primary care
  • Experience of cardio exercise programmes
  • Experience of previous service development

Skills

  • Willingness to undertake further training and education as required
  • Research awareness
  • Venepuncture or willingness to work towards it
  • Clinical assessment skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Ability to delegate, supervise, and support colleagues
  • Conflict management skills
  • Ability to give and receive constructive criticism
  • A flexible approach to working hours
  • Current valid driving license
  • Audit and research awareness
  • Experience of project management
  • Networking skills as appropriate

Knowledge

  • Understanding of Information Governance and Confidentiality
  • Cultural awareness, with experience working with ethnic minority groups

Qualifications

  • Registered Nurse (Level 1)
  • Current NMC registration
  • Relevant qualification or training in CHD
  • Relevant teaching and assessing qualification
  • First Degree
  • Independent Nurse Prescribing qualification (or willingness to work towards)

Salary: £37,338 to £44,962 per annum (pro-rata)
