Job overview

The MLCSU Personalised Healthcare Commissioning (PHC) Team based in Leicestershire is committed to ensuring excellent healthcare for all individuals.

We are seeking a motivated and experienced Learning Disabilities (RNLD), Mental Health (RMN), Registered Adult Nurse (RN) who can work autonomously and collaboratively in a multi-agency environment. The role involves supporting assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC), including comprehensive person-centred assessments to determine eligibility.

The ideal candidate will have a good understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, excellent communication and problem-solving skills, and the ability to produce high-quality documentation while working to deadlines and targets.

Main duties of the job

  1. Assess, review, and actively case manage care home with nursing clients and individuals with Learning Disability / Mental Health needs, within their own homes / supported living, who are considered eligible for NHS Continuing Care and funded nursing care contribution (FNC).
  2. Work closely with local partners, including health, social care, and independent providers, in planning, managing, and reviewing patient care needs. Provide clinical advice regarding NHS Continuing Care and NHS Funded Nursing Care (FNC).
  3. Progress the delivery of care under the Personal Health Budget agenda.
  4. Liaise with internal and external organizations and lead on the progression of Transition cases through the Continuing Healthcare process.

Key Responsibilities

  1. Lead complex cases within the Personalised Healthcare Commissioning Assessors team, supporting and mentoring staff to ensure high-quality service delivery.
  2. Manage a caseload of patients, contributing to performance improvement and taking a lead in specific areas.
  3. Manage staff in accordance with HR policies.
  4. Facilitate and contribute to multidisciplinary assessments and reviews.
  5. Participate in recruitment and team development activities.
  6. Support the team to meet workstream goals and ensure compliance with relevant frameworks.
  7. Deputize for the Clinical Team Leader as needed.
  8. Assist with handling complaints, appeals, and disputes.
  9. Mentor and supervise team members and students.
  10. Monitor workstream performance and quality assurance processes.
  11. Investigate variances from targets and implement solutions.
  12. Communicate effectively regarding risks, issues, and dependencies.
  13. Maintain professional registration and knowledge of legislation.
  14. Provide expert advice on functional and information matters.
  15. Prioritize workload and meet deadlines, ensuring quality outcomes.
  16. Promote transparency and a person-centered approach.
  17. Encourage the use of Personal Health Budgets for eligible individuals.
