Job description

Reporting to the Colorectal Nurse Manager - Leicester, you will deliver high quality, evidence-based care to all patients with colorectal disease. You will also provide expert advice and education for all staff within the hospital and community. You will be responsible for reviewing, auditing and improving the quality of care delivered. This role encompasses clinical, managerial, educational and professional responsibilities, in collaboration with a multidisciplinary team.

In order to meet the overall mission of the SecuriCare and UHL the post holder will:

• Deliver safe and effective patient centred services, which meet national, regional and local requirements for patients with a colorectal cancer and other related colorectal disease under the direction of the Colorectal Nurse Manager.
• Support development of the nursing service to achieve person centred care under the direction of the Colorectal Nurse Manager
• Provide clinical expertise within the specialist area of colorectal cancer and other related colorectal disease.
• Ensure the co-ordination of patient care across the clinical pathway in conjunction with the multi-professional team.

Job Summary

• Act as a clinical expert and professional resource to the patients and carers and multi-professional team within the specialist service.
• Work alongside other Specialist Nurses / Head of Nursing to ensure the development and delivery of a high-quality service.
• Directly or indirectly influence all aspects of care and management of patients within the service/specialty.
• Deliver high quality, clinically effective care, and patient experience, utilising research in collaboration with colleagues in everyday practice and assist with the smooth running of the department’s research.
• Act as a role model and clinical resource demonstrating high standards of care and providing clinical leadership in the specialist area.
• Demonstrate good professional and organisational skills to help ensure that trials and projects run to time and can meet their objectives.
• Contribute to the development of the department’s research programmes by undertaking an active interest in, and an awareness of, current advances in the field.
• Employ good communication and interpersonal skills with study participants, patients, carers and key members of the research and clinical teams to promote effective working practices and study outcomes.

Patient Care, Clinical Expertise and Case Management

The Clinical Nurse Specialist is an experienced registered nurse with specialist skills to assess and treat patients and manage the clinical environment, in liaison with consultant team. After appropriate training and assessment, the nurse will act as an autonomous practitioner in forming clinical decisions and complex management plans in the assessment and treatment of patients. The post holder will work closely with the multidisciplinary team and collaborate with colleagues to ensure that high quality patient care is maintained through:

• Managing a delegated caseload of patients, as appropriate within the speciality delivering individualised and personalised direct/ patient care and advice and support through telephone contact with patients
• Demonstrate a desire to develop in-depth specialist knowledge, skills and innovative practice in the specialist area, ensuring the delivery of appropriate evidence-based care.
• Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This will necessitate high levels of autonomy and advocacy.
• Contribute to the smooth running of the research studies where relevant
• Ensure that records and data are recorded accurately and promptly according to agreed protocols and be involved with the presentation of data.
• In conjunction with senior nursing and consultant staff formulate the development of clinical guidance for all aspects of clinical practice within the clinical specialty

Promote and Establish Effective Team Working and Communication

Take responsibility for ensuring effective communication between all service providers within the specialty.

Establish and maintain good positive working relationships with the Heads of Nursing, Sister / Charge Nurse and their teams, Education and Practice Development Lead, Matrons, relevant Clinicians, Service and General Managers.

Education, Training, and Practice Development

Provide specialist education and training to nursing staff within the CMG in collaboration with the Stoma team and Colorectal Nurse manager.

Actively participate in lifelong learning and personal development through annual appraisal and development.

Promote and implement patient focused education and the information giving strategy within the speciality.

Work with the Colorectal Nurse Manager to ensure that specialist education and training meets the needs of the service, multi-professional clinical teams, patients and carers.

Demonstrate continuous evaluation of practice and make changes where appropriate, within the defined speciality and in conjunction with cross-site colorectal team.

Use Education forums and Networks proactively sharing good practice and innovation with others e.g. attendance at Coloproctology and ASCN.

Liaise with other groups and assist with the delivery of multi-professional and inter-professional learning.

Provide specialist education teaching, learning and mentoring for pre and post registration students when required and actively contribute to the delivery of the Universities curriculum.

Keep abreast of research evidence.

Clinical Governance, Audit, and Research

• Contribute to policy making and clinical guidelines, internally and externally, necessary to support the specialist nursing service in conjunction with UHL and SecuriCare clinical governance frameworks.
• Actively participate in audit and research and benchmarking activities to ensure effective safe and evidence-based practice.
• Provide and promote clinical excellence that reflects the Trust’s and SecuriCare’s Clinical Governance Strategies.
• Promote the specialty by publishing and presenting innovations and research findings locally and nationally.
• Actively promote the specialist service throughout the Trust by establishing robust networks and links with Clinical Governance and Audit teams.
• Act on audit and research findings to inform and change practice as appropriate.

Risk Management and Patient Safety

• Use specialist knowledge, skills and innovative practice in order to provide a high standard of patient care and maintain patient safety.
• To ensure NMC Code of Conduct and SecuriCare’s professional guidelines are applied to practice.
• To refer to other practitioners when health outcomes or risks and needs are beyond own competence and scope of practice.
• To take appropriate action to safeguard patients/clients under his/her care and involving a Colorectal Consultant when necessary.
• Actively promote infection prevention and control awareness using best practice to reduce the potential for cross infection.
• Deliver new ways of working practice that benefits patient safety and the patient user experience.
• As a clinical expert provide advice and support to assist with the management of complaints relating to the speciality and implement action plans or change of practises in conjunction with Clinical Governance Lead for SecuriCare.

Specialty Specific

• Provide highly specialist advice and support regarding all aspects of colorectal disease interventions and treatments to patients and their families and carers and support.
• To Manage a sizeable patient caseload
• To run Nurse-Led Clinic providing continuity of care, symptom management, surveillance of disease and psychological support, following protocols for investigations to facilitate early detection of recurrent or metastatic disease.
• Provide health education to groups of patients and individual patients to ensure that appropriate, consistent realistic information is provided to patients and families. Provide sensitive and complex information to patients/carers in sometimes hostile and highly emotive atmospheres.
• Ensure good communication with MDT.
• Make and receive direct referrals to and from the relevant members of multi-professional teams, and primary care colleagues.

Safeguarding Children and Vulnerable Adults

The post holder is responsible for safeguarding the interests of children and adults who they come into contact with during their work. To fulfil these duties post holders are required to attend training and development to recognise the signs and symptoms of abuse or individuals at risk, to follow local and national policy relating a safeguarding practice and to report and act on concerns that they may have.
