Overview

An exciting opportunity has arisen within the Colorectal Cancer Nurse Specialist team as a Cancer Care Coordinator. The role involves supporting the Clinical Nurse Specialist team with the delivery of Personalised Care for Cancer. Personalised Care for Cancer includes supporting the provision of Holistic Needs Assessments, developing a supportive care plan, and offering Health & Wellbeing advice and support. The role also involves supporting the implementation and administration of Personalised Stratified Follow-Up, monitoring and tracking of surveillance tests, and acting as a single point of access for patients. This will include rapid re-entry into the system for those identified as having urgent or specialist needs. You will work closely with all grades of medical and nursing staff and wider health care teams. The role requires a team player who can also work independently, with excellent organisation, IT and interpersonal skills, and a passion for ensuring compassionate care for people living with and affected by cancer. Effective daily communication with patients, relatives, carers, other health/social care professionals and community workers is essential.
