A leading NHS Trust is seeking a dedicated Clozapine Mental Health Nurse to join its innovative tri-borough service. This role involves monitoring the health of patients prescribed clozapine, leading clinics, and collaborating with various healthcare teams to deliver integrated care in the community and inpatient settings.
Location: Primarily based in H& F (0.6 WTE ) & Ealing (0.4 wte), with occasional support in Hounslow (the role is a split role)Hours: Full-time, 37.5 hours per week (split role) (secondment - 12months )

We are looking for a committed Clozapine Mental Health Nurse to join our innovative and supportive tri-borough Clozapine Service, covering Ealing, Hammersmith, and Hounslow occasionally.
About the Role:As a key member of the Clozapine Service, you will play a vital role in delivering high-quality care and physical health monitoring for patients prescribed clozapine--both in the community and inpatient settings. You'll help coordinate and lead clozapine clinics, liaising closely with patients, carers, community teams, GPs, inpatient units, and Consultant Psychiatrists.
What You'll Do:? Monitor patients' mental and physical health? Coordinate and support the running of clozapine clinics? Collaborate across multidisciplinary teams to ensure seamless care?
