Job summaryClinical Triage Sister, Integrated Single Point of Access (ISPA) - 18.5 hours per weekAre you looking for a new and exciting challenge? This is a great opportunity to join one of our Integrated Single Point of Access Team. We are looking for experienced and motivated clinicians with a nursing qualification and a keen interest and/or experience in triage.
This role is being established following a review as part of our transformation programme. There will be a multi-disciplinary workforce in the ISPA alongside our experienced administration team.
You will be responsible for the triage and prioritisation of all new referrals as well as the prioritisation of urgent community response and hospital at home referrals. You will require expert clinical knowledge and judgement to facilitate your decision making.
The roles will be office based with some clinical time included, and we can offer flexible working to support your work life balance. This service will operate at various clinical bases 7 days a week, 24 hours shift pattern.

The post holder will use their sound clinical knowledge and risk assessment skills to prioritise referrals through the clinical triage service as necessary.
Main duties of the jobhe post holder will support the provision of the Spa MDT and will provide senior clinical input to ensure patients are signposted to the appropriate services.
The post holder will ensure that all contacts with the clinical triage services reach an effective and appropriate outcome for the patientThe successful candidate need to be prepared to adapt to the changing needs of the Trust and the community models of care as they arise.
