Lancashire & South Cumbria NHS Foundation Trust is seeking a Band 6 Registered Nurse or Allied Health Professional to join Care Connexions, delivering timely, joined-up community care and triage to direct patients to the right service.

You will work within a multidisciplinary team, coordinating referrals and coordinating care planning to avoid hospital admissions and support people at home across 7 days, 08:00–20:00 hours.

Qualifications

• Registration with the appropriate professional body is essential.
• Diploma or degree in nursing or allied health.
• Evidence of continuing professional development in the area of specialty.

We are seeking a motivated and enthusiastic Band 6 Registered Nurse or Allied Health Professional to work within Care Connexions. This role offers an excellent opportunity to be part of an innovative service supporting timely, joined‑up care for patients in the community, with a strong focus on hospital avoidance and ensuring people receive the right care, in the right place, at the right time. The service currently operates across 7 days, 08:00hrs – 20:00hrs

Main duties of the job

This exciting role will support the ongoing development of an integrated community response model, working in partnership with acute trusts, primary care, Northwest Ambulance Service, the voluntary and third sector, Social Care, therapy services and wider community teams.

The role will focus on clinical triage, ensuring referrals are responded to in a timely and effective way so that patients are directed to the most appropriate service. You will work closely with a Multi‑disciplinary team consisting of Advanced Clinical Practitioners, Nurses, Therapists and Assistant practitioner to support safe, coordinated care planning.

Person specification

Education

• Appropriate Professional Registration
• Diploma or Degree in Nursing or Allied health professional/Health related subject.
• Evidence of continuing professional education or development in area of speciality

Knowledge

• Specialised knowledge in discipline to ensure effective and accurate triage and assessment of patients
• Significant experience in working in an MDT environment, liaising with multiple service providers ensuring appropriate care provision and planning
• Post degree learning and experience in relevant area
• Evidence of leadership experience and learning
