Job Summary:

The Providence Center Child & Family Services Clinical Therapist will provide outpatient psychotherapy either at The Providence Center or in the community. Perform a comprehensive psychosocial assessment of assigned consumers.

Duties & Responsibilities:

1. Complete crisis assessments, initial assessments, and integrated summary forms.
2. Develop and implement a plan of care with ongoing consumer input.
3. Provide individual mental health and/or substance abuse treatment and crisis assessment.
4. Perform record keeping in accordance with Health Information Services and other Center requirements.

Requirements:

1. Master’s degree in Humans Services, Psychology, Social Work or related field and independent license required (LICSW and/or LMHC).
2. Must have insured auto and valid driver’s license.
