A leading healthcare provider is seeking a Clinical Team Leader for its Hospital@Home service. This part-time role (15 hours per week) involves managing day-to-day operations, ensuring safe patient transitions from hospital to home, and providing quality care for children with acute illnesses. Significant paediatric nursing experience is required. The position is fixed-term for 12 months to cover maternity leave.
We are seeking a dynamic and enthusiastic candidate to join our Paediatric Hospital@Home service on a 12-month fixed term contract for Maternity Leave cover as Clinical Team Leader. This role is offered at 15 hours per week as part of a job share; the successful candidate will work 2 shifts per week as part of a roster agreed with their line manager.
The Hospital at Home (H@H) service is commissioned by NCL ICB and has provided positive patient experience and outcomes to children in their own homes, who otherwise would have been admitted to inpatient units. The function of the team is to provide hospital-standard care to children with acute nursing needs in the community. Working within a virtual ward model, the service will develop bespoke care pathways for children requiring nursing care, which will avoid extended lengths of stay and admission. The team will work closely with paediatricians and pharmacy colleagues to ensure a seamless transfer home and provide high-quality nursing care according to their treatment plan. The BCYP H@H operates 7 days per week, 08:00 to 20:00. The service currently offers 7 care pathways and continues to develop new pathways. The Hospital at Home service sees babies, children and young people at home for treatment and assessments.
The post holder will be supported to manage the H@H Service, implement pathways and lead on the day-to-day running of the service. They will work within the multidisciplinary team to provide safe and appropriate management of children and young people with an acute illness that can be treated in the community. They will work closely with acute teams to ensure patient safety.
