Job description

This post will involve competencies which will need to becompleted within 2 years dependent on current experience and will be assessedthrough various methods including feedback, self-directed assessment, directobservation and throughout line management. If not achieved within thespecified timeframe, the postholder will not be able to progress onto the band6.

The postholder isresponsible for the assessment of care needs and the development,implementation and evaluation of programmes of care.

He/she will bedesignated to take charge regularly of the ward.

The postholder isrequired to supervise staff and be able to teach qualified and unqualifiedstaff, including basic and/or post basic students.

TheClinical Team Leader will at all times work within the spirit of the NMC codeof Professional Conduct for trained nurses and local policies.

All Livewell Southwest staff are expected to able and willing to work across a 7 day service.

Main duties of the job

To be a member of a multi-disciplinaryteam, contributing in all its forums, attending meetings and case conferencesfor patients.

To link closely with Key Workers in the delivery of care under the Personalised Care Framework.

To promote a collaborative approach topatient care within the team providing support to junior staff.

To assist the Ward Manager in theintroduction Quality Improvement Projects and Culture of Care Project and in the introduction of an effective care delivery system devised to meet the needs of patients which is evidenced based wherever possible.

To act as clinical role model,demonstrating high standards of nursing practice.

To assist the Ward Manager and DeputyWard Manager in the introduction of patients involvement in the day to dayissues on the ward.

To ensure close links with otherGlenbourne wards/departments to identify/resolve clinical issues.

To be responsible implementing drugadministration procedures according to laid down policies.

Actively participate in clinicalgovernance and quality improvement initiatives, e.g. clinical audit, essence ofcare, or clinical policy development.

Provide, receive and analysis complexinformation from a range of sources.

Analysis and make decisions in relationto patient risk.

Job responsibilities

Education:

To act as assessor/supervisor forlearner nurses.

To ensure the provision of a stimulatinglearning environment for nurses.

To provide teaching programs for theeducation and interest of nurses in the team, in conjunction with theUniversity.

To include a health and wellbeingeducation component to the core group program for patients on the ward.
