Job description

We are looking for a highly motivated ‘can do‘ individual to join our Virtual Ward Team.

An exciting opportunity has arisen to help with the expansion of the virtual wards across Walsall. You will be responsible for assisting the practitioner in charge of the shift with the management of patients receiving care on the virtual ward. You will need to be competent in the use of computer systems and have experience in the delivery of high-quality patient centered care in the acute setting or the patient's own home.

You will need to be able to work flexibly to meet the demands of the service, working 7 days across a 7 day service that will operate 24/7. You must demonstrate excellent communication skills and have experience of multi-disciplinary/multi agency working.

Key responsibilities include:

  • Visiting patients in their own home to support with care/clinical treatment they require.
  • Reporting findings and acting on team leader's instruction.
  • Supporting the team leader with promotion of the service.
  • Supporting the team leader with data collection and inputting.
