Our 21 bedded unit provides rehabilitation to a wide group of patients with varying complexities of needs. Our patients come to us following medical or surgical treatment or if appropriate, directly from their own homes. We also provide palliative and end of life care.

Opportunities exist for further training and development within the role. You will be joining an enthusiastic, supportive, friendly multidisciplinary team.

The post involves rotation between day/night duties, full/ part time posts are available. Our focus is on continually improving the quality of care we offer to local people and on improving access to that care, helping people to move seamlessly from one service to another.

Responsibilities
  • Implement care for patients as defined by the individualised care plan.
  • Provide a high standard of direct care for patients.
  • Work in a collaborative manner with all other staff working within the unit.
  • Establish a good relationship with the patients and provide an appropriate support mechanism so the patients are able to express their needs.
  • Ensure the patients are involved in the decision making process, communicate all relevant information regarding the needs of the patients, family and friends.
  • Document and sign all care interventions provided.
  • Be involved in the induction and mentoring of new staff where applicable.
Key Duties
  • Ensure effective and efficient use of all ward/department resources.
  • Maintain a positive approach towards achievement within the team.
  • Ensure adequate provision and maintenance of equipment.
  • Assist with the induction of new staff and students, acting as a mentor.
  • Act as a role model giving guidance and support to junior staff and students.
  • Participate in problem solving that is related to your work and organisation.
  • Ensure that patients and carers are aware of their current and ongoing health needs.
  • Participate in and promote clinical supervision.
Requirements
  • To accurately and promptly carry out the instructions of the Registered Nurse (RN) / Registered Nursing Associate (RNA) who is professionally accountable for the delivery of care.
  • To carry out duties as specified within the trust’s Approved Core Task List for Clinical Support Worker (working within assessed competence).
  • To report to registered staff any patient that gives cause for concern or any observed changes in a patient’s clinical condition.
  • To support patient’s emotional well-being by supporting positive conversations, actively encouraging participation in ward-based activities.
  • To accurately document all care delivered to patients, including recording documentation electronically on trust systems maintaining confidentiality at all times.
