Clinical Support Worker - Mental Health Crisis Assessment Service

MAIN DUTIES AND RESPONSIBILITIES

Clinical/Practice

  • Follow plans of care and undertake a range of allocated duties related to direct or indirect service user support and care.
  • Care for the service user's physical, psychological, social, spiritual, cultural, and recreational needs within a defined healthcare program.
  • Ensure that service users are treated with respect and their dignity is maintained at all times.
  • Encourage service users to accept responsibility for their care and, with their consent, involve carers, relatives, and friends.
  • Observe and report any changes in the service user's wellbeing and undertake routine observations for physical health management.
  • Maintain accurate and timely service user records in line with trust and service policies.
  • Communicate positively and respectfully with service users and their families, assessing their needs and providing appropriate assistance.
  • Participate in assessment processes, including data collection and monitoring patient progress, and report findings clearly during team reviews.
  • Contribute to the ongoing quality of service and care, identifying any risks related to the service user's health and social care.
