Join Our New 24/7 SPUR Service as a Clinical SpecialistTriage & Facilitator Nurse

AtBirmingham Hospice, our patients are at theheart of everything we do. From our Inpatient Units to our Living Well Centresand bereavement counselling services, we are here for our community when theyneed us most.

We are launching our new24/7 Specialist PalliativeUrgent Response (SPUR)service, and this role will operatebetween8:00 PM and 8:00 AM, responding to urgent palliative careneeds across Birmingham and Solihull.

As aClinical Specialist Triage & FacilitatorNurse, you will be the first point of contact for all incoming calls to theservice. A clinical leader, you will work autonomously and collaboratively withother healthcare professionals to assess, diagnose, and treat patients in theirplace of residence, ensuring timely and appropriate clinical responses.

You will provide expert palliative care support to patientsand their families, offering sensitive communication and addressing theirevolving physical, psychological, and spiritual needs. You will also beresponsible for delegating clinical responsibilities, liaising with externalservices, supporting medical emergencies, and managing safeguarding concerns.

Job purpose

TheSpecialist Palliative Response Service (SPRS) will be delivered between thehours of 20:00 08:00.

Duringthese hours the Triage/Facilitator Nurse, through the triage and clinicalrotation (e.g. 1 week of triage, 1 week of clinical) will be to respond tourgent palliative response calls from patients, their families/loved ones andprofessionals within the Birmingham and Solihull area, and provide specialistpalliative and end-of-life support and advice either through delegation tosatellite teams as the triaging nurse or in person as the clinical specialist. Visits will aim to take place within a 2-hourtimeframe.

Therole will require you to:

Act asan autonomous practitioner working independently and in conjunction withother health professionals.

Assess,diagnose and treat the conditions of patients within their own homes andinitiating referrals as appropriate.

Independentlyprescribe appropriate medication, evaluate or refer to other specialists ifnecessary.

Workcollaboratively with the hospice, community, primary care and acute teams, tomeet the needs of patients and support the delivery of policy and proceduresand provide clinical leadership.

Provideexpert professional advice to patients, carers and colleagues and ensure themaintenance of clinical excellence.

Co-ordinateall available staffing resource, working with the heads of service to deliverpriorities.

Provideeducation and training to other staff and students.

KeyResponsibilities

Prioritiseand triage patients presenting with a range of conditions includingpalliative, End-of-Life and acute conditions, making any necessary referralsin an appropriate manner

Implementand evaluate personalised care planning, individual specialised treatmentplans, MASC, RESPECT and advanced care planning for patients

Providespecialist palliative care and support to patients and their families/lovedones in the place they call home as required in accordance with clinicalbased evidence, NICE and the NSF

Besensitive to patients and their families/loved ones changing physical,psychological and spiritual needs

Processand interpret pathology and other diagnostic results as required

Maintainaccurate clinical records in conjunction with current legislation

Supportthe team in dealing with clinical emergencies

Supportpatients in the use of their prescribed medicines or over the countermedicines (within own scope of practice), reviewing as required

Liaisewith external services/agencies and the patients usual community teams toensure the patient continues to be supported appropriately

Delegateclinical responsibilities appropriately (ensuring safe practice and the taskis within the scope of practice of the individual)

Supportthe team with all safeguarding matters in accordance with local and nationalpolicies

Supportand participate in shared learning within the system

Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate

Participatein the review of significant and near-miss events applying a structuredapproach (PSIRF learning)

Ensurethe clinical team maintain high quality standards for the environment,medical equipment and infection control issues in line with CQC standards andNICE guidance and follow local policies and procedures

Allemployees are responsible for ensuring they undertake any training relatingto information governance, read the hospices policies, procedures andguidance documents relating to information governance, and understanding howthis affects them in their role.

Allemployees must participate in an annual appraisal and develop a personaldevelopment plan with their line manager.

Allemployees are responsible for maintaining their statutory and mandatorytraining.

Safeguarding children, young people and vulnerableadults

The hospiceis committed to safeguarding and promoting the welfare of children, youngpeople and vulnerable adults. All employees and volunteers are expected tobehave in such a way that supports this commitment pandemic or major incident.

In theevent of a pandemic or major incident, the post holder may be asked toundertake other duties not necessarily commensurate to the banding of thisrole. This could include duties in any part of the hospice. Prior toundertaking any duties, the member of staff will have full training andinduction. We wont ask any member of staff to undertake duties for whichthey are not competent or where they feel unsafe in their environment orcould put patients or themselves at risk.
