Job description

Assess patients' requirements in relation to their goals. Plan, manage, and review functional and psychological restoration for individuals and groups. Review care plans at timely intervals and communicate effectively with the patient's GP, advising on ongoing and flare-up management plans agreed with the patient. Provide safe, effective, and dignified care according to best practices and national guidelines. Provide advice and treatments to individual patients within best practices and national guidelines.

Main duties of the job

Conduct comprehensive assessments, diagnosis, and management of patients with non-inflammatory or persistent joint problems, traditionally referred to secondary care rheumatology services. Develop and implement individualized treatment plans, including pharmacological and non-pharmacological interventions, to manage patients' conditions effectively. Provide specialty advice to stakeholders across the care pathway. Lead and participate in multidisciplinary team meetings to coordinate care and ensure holistic treatment approaches. Maintain professional relationships with external clinical stakeholders such as secondary care consultants and GPs to facilitate successful partnership working and high-quality rheumatology care delivery.
