London North West Healthcare NHS Trust is seeking a pharmacist to lead and advance clinical pharmacy services across acute wards, under the direction of the Chief Pharmacist and Clinical Services Manager.

You will deliver, develop and evaluate services with multidisciplinary teams, contribute to guidelines and audits, provide ward-based education, and support the dispensary to ensure safe, cost-effective medicines use.

Responsabilidades

• To lead, deliver, develop and evaluate general clinical pharmacy services in accordance with the objectives and priorities agreed between the post holder, Chief Pharmacist and Clinical Services Manager in order to ensure safe, clinically effective, and cost efficient use of medicines.
• To be a valued member of the clinical pharmacy team, work closely with the multi-disciplinary teams to provide a consistently highly specialist pharmaceutical service to patients on acute wards.
• To provide a consistently highly pharmaceutical service for patients within a range of areas within the acute hospital wherever needed.
• To be able to work as part of a multidisciplinary team within the department and wider teams to ensure optimal medicines usage and value for money.
• To provide a framework for the safe, efficient and cost effective use of medicines within the Trust, by developing procedures, policies and guidelines, and participating in clinical audit.
• To participate in multidisciplinary ward-based education and training of junior doctors, pharmacists and nursing staff.
• To provide daily ward visits to acute wards as well inputting on appropriate guidelines and attend MDT ward rounds.
• To provide regular support to the dispensary.
