East London NHS Foundation Trust is seeking a Clinical and Service Lead to oversee the NHS Continuing Healthcare framework for Tower Hamlets and Newham ICB-commissioned clients. You will line-manage the CHC team and drive delivery of care assessments and KPIs.

You will provide expert clinical guidance to the Adult Continuing Care Panel, coordinate case management, and ensure timely, fair assessments and placements across complex funded packages.

• Provide clinical guidance to the Adult Continuing Care Panel and coordinate case management.
• Lead the CHC team to deliver timely and fair assessments and funded nursing care.
• Promote seamless care provision for Tower Hamlets and Newham residents.

Aufgaben

• Lead the CHC Team in assessment and review of eligibility and funded nursing care reviews.
• Coordinate multidisciplinary involvement to ensure cost-effective placements.
• Deliver NHS Continuing Healthcare training across health and social care.
• Respond to complaints arising from eligibility decisions.
• Act as adult safeguarding lead within the service.
