Job summary
Are you looking to be part of an established, high-performing service that delivers anticoagulation treatment to patients in their homes, community settings, and acute hospitals?
This is an exciting opportunity to step into a new Clinical Lead role within our service. You will work within the Community Medicine and Ambulatory Therapies Directorate of the Primary Care Communities and Therapies group--a dynamic, forward-thinking, and supportive team committed to excellence in patient care.
This role is ideal for an individual looking to expand their knowledge and skills while contributing to the redesign of our Anticoagulation service. As Clinical Lead, you will work alongside the Hematology Consultant and the wider team to implement and embed new service developments, promote the service, and drive improvements in patient care . You will play a key role in developing new pathways and policies to enhance quality and safety by streamlining inpatient and community outpatient care. Through collaborative work with multidisciplinary teams, you will help optimise patient outcomes and reduce hospital length of stay.
Main duties of the jobMain duties
Participate, drive, and contribute to continuous service improvement and development.
Lead quality improvement projects to ensure the agendas for the service are achieved and maintained.
Demonstrate leadership, flexibility, the ability to support junior member of the team, with the ability to build strong working relationships.
Actively promotes excellent and collaborative team work across the multidisciplinary team with outcomes aimed at the best interests of the patients.
Maintain a high level of clinical practice within the service.
Ensure there is an imbedded clinical governance framework across the service and compliance with reports and audits.
