Job summary
Overall responsibility for the management of the Datix system and co-ordination of the serious incident processes.
Act as a central point of expertise to advise and support the Healthcare Groups in managing incidents reporting processes, escalation and external reporting.
Main duties of the jobClinical Safety Manager is an experienced senior practitioner with clinical expertise who is able to interact with all levels of clinical and management teams.
Co-ordinates the incident process, including liaison with external bodies such as the Integrated Care Board (ICB) and Trust Corporate and Divisional teams, acting as key point of contact to support appropriate and timely reporting in line with local and national guidelines.
Overall responsibility for the management of the Datix incident reporting system; an expert user facilitating training and access for end users.
Key point of contact for designated Corporate / Divisional teams; providing cover and support for these groups outside of direct remit when the identified lead is not available.
Line management responsibility for the Clinical Safety Team/ or provision of cover and support in the absence of line manager
Overall responsibility for the co-ordination and facilitation of the Incident Management Group (IMG) and Patient Safety Incident Assurance Panel (PSIAP) meetings to ensure completion of process and supporting corporate team to manage their incidents.

Be an expert Datix user and a specialist in incident investigation techniques and maintain the incident management process ensuring that Corporate and Divisional teams comply with National and local incident reporting policy.
Organise the twice weekly Incident Management Group (IMG) and twice monthly Patient Safety Incident Assurance Panel (PSIAP).
Act as key link between Divisional teams and the Integrated Care Board (ICB) on all incident related matters and be responsible for communicating with the ICB when incidents occur that meet our PSIAP Framework criteria.
Monitor the incidents under investigation; including support and feedback to Divisions and lead investigators to ensure an appropriate standard of written reports, timely review, reporting and monitoring of Action Plan tracker.
Liaise with appropriate internal and external stakeholders to support realisation of key actions arising from IMG / PSIAP.
Monitor non-externally reportable significant incidents including support and feedback to Divisions and lead investigators to ensure an appropriate standard of written reports, timely review, reporting and monitoring of action plans.
Overall responsibility for the development, management and analysis of information from the Datix system; facilitating key individuals to retrieve quality data on a daily basis
Act as a clinical resource for the divisional Quality and Safety teams and, in the absence of the Patient Safety Specialist, be the main point of contact for clinical advice/support.
Establish mechanisms to bring together key clinical information into trends and support the Patient Safety Specialist in making recommendations for improvement or change to minimise risk.
Be responsible for the provision of monthly reports on incident status and themes, for key internal and external groups.
Working with the Patient Safety specialist, support the development, quality assurance and delivery of Trust Induction Training on relevant incident reporting and safety; including regular updating of package to reflect current policy and legislation.
