Portland Medical Centre, as part of Bourne Health, is seeking a proactive and enthusiastic Pharmacist with prescribing qualifications. Preference will be given to candidates with hospital pharmacy experience and expertise in diabetes to work closely with our nurses and GPs.
The appointed Pharmacist will be part of a wider pharmacy team working within a Hub model alongside our associate practices.
The role involves developing responsibilities in Chronic Disease Management, conducting Clinical Medication Reviews, and proactively managing patients with complex poly-pharmacy needs. It also includes providing primary support to GP staff regarding prescriptions (including repeat prescriptions), medication queries, and complex clinical issues. Training will be provided to manage low-level investigation results following strict protocols, with mentorship from a GP Partner.

All staff must uphold confidentiality, data protection, and information governance policies. Responsibilities include maintaining patient and staff confidentiality, complying with health and safety, infection control, risk management, and safeguarding policies. Flexibility in working hours and locations is expected, along with a commitment to sustainability and non-smoking policies.
This role requires a DBS check due to the Rehabilitation of Offenders Act 1975.
Salary range: £55,000 - £60,000 per annum, depending on experience.
