Job description

Portland Medical Centre, as part of Bourne Health, is seeking a proactive and enthusiastic Pharmacist with prescribing qualifications. Preference will be given to candidates with hospital pharmacy experience and expertise in diabetes to work closely with our nurses and GPs.

The appointed Pharmacist will be part of a wider pharmacy team working within a Hub model alongside our associate practices.

Main duties of the role

The role involves developing responsibilities in Chronic Disease Management, conducting Clinical Medication Reviews, and proactively managing patients with complex poly-pharmacy needs. It also includes providing primary support to GP staff regarding prescriptions (including repeat prescriptions), medication queries, and complex clinical issues. Training will be provided to manage low-level investigation results following strict protocols, with mentorship from a GP Partner.

Job responsibilities

Key responsibilities include:

  1. Performing virtual medication reviews for repeat prescriptions, care homes, and pharmacies.
  2. Responding to clinical medication queries from GPs, nurses, and other healthcare professionals.
  3. Handling complex medication queries from patients.
  4. Collaborating with secondary care teams on medication issues.
  5. Reviewing hospital discharge letters and conducting medication reconciliation.
  6. Managing complex community pharmacy medication queries.
  7. Conducting medication safety audits and recommending process improvements.
  8. Processing shared care medication requests from specialists.
  9. Conducting face-to-face medication reviews when indicated.
  10. Responding to MHRA safety alerts.
  11. Prescribing and managing repeat and acute medications.

This list is not exhaustive; team members should be adaptable to additional duties to ensure efficient practice operation.

Internal: Senior Management, Partnership Board, colleagues within The Bourne Partnership.

External: Service providers, other GP practices, external organizations.

Other considerations

All staff must uphold confidentiality, data protection, and information governance policies. Responsibilities include maintaining patient and staff confidentiality, complying with health and safety, infection control, risk management, and safeguarding policies. Flexibility in working hours and locations is expected, along with a commitment to sustainability and non-smoking policies.

Person specification
Experience
  • Supervising junior pharmacists/technicians (preferred).
  • Experience in Asthma or COPD reviews.
  • Experience or willingness to train in Diabetes reviews.
Qualifications
  • Registered with the GPhC.
  • Completion of PCN CPPE training.

Disclosure and Barring Service Check

This role requires a DBS check due to the Rehabilitation of Offenders Act 1975.

Salary range: £55,000 - £60,000 per annum, depending on experience.
